For your convenience, we accept single requests for an issue of the current or upcoming issue over the Internet. An "On-line Single Issue Request" can be submitted to the journal using the form that appears below. Sorry: we are unable to accept or process payment via the Internet. Your order will be processed as follows:
  1. An "On-line Single Issue Request" can be submitted to TLCP using the form that appears below.
  2. When received, the E-mail message containing your order will be transcribed to a standard (paper) Request Form.
  3. The Request Form will then be mailed to you within five business days (or sent via facsimile if a number is provided).
  4. Payment preferences, credit card numbers, purchase order numbers, etc., should be provided via the Request Form when it arrives.
  5. When payment preferences and your order request have been confirmed, the Request Form can be returned to TLCP via mail or facsimile, where it will be promptly processed and recorded.

The following form may be used to submit a single issue request to TLCP. For your order to be promptly processed, we ask that you complete all fields (except those marked "optional"). All information obtained will remain confidential and will be used only for the purpose of filling your order.
